The Massachusetts Chapter of the American College of Health Care Administrators wants you to attend the National Convocation in Covington, KY/Cincinnati, OH.
This year’s annual conference & exposition will be from May 16-20, 2008 at the Northern Kentucky Convention Center in Greater Cincinnati, OH.
The Massachusetts Chapter will reimburse 100% of the “early bird” registration fee for MA Chapter members who register. $599.00!!
To be eligible to receive your reimbursement, follow this three step procedure:

Declare your intention to attend by entering your name and membership number on this form and fax it to 508-655-9702.

After you have registered for the Convocation and paid, fax your registration form and proof of payment to 508-655-9702.

After the Convocation, fax proof of attendance (CEU sheet) and this form to 508-655-9702. (deadline July 31, 2008)
